Bugs found by red team audit (2026-02-20)

  1. Automation race condition — race detector fires (filed in ash/ideas #15)
  2. CommandBus context cancellation broken (filed in ash/ideas #17)

Note: GDPR clearPII panic and batch decryption bugs were already fixed by red team.

Must fix before v1.
